Terms of Service

Effective Date: January 1, 2025  |  Greater ATL Health — Lyna Ashu, FNP-C

By using our website or scheduling a telehealth appointment, you agree to these Terms of Service. Please read them carefully before booking.

1. Acceptance of Terms

These Terms of Service ("Terms") govern your use of the Greater ATL Health website (greateratlhealth.com) and all telehealth services provided by Lyna Ashu, FNP-C ("Provider"). By accessing this website or booking an appointment, you agree to be bound by these Terms and our Privacy Policy.

2. Nature of Services

Greater ATL Health provides telehealth medical services via secure video visits, including but not limited to primary care, preventive care, DOT physicals, sports physicals, weight loss management, and treatment of acute illness and minor injury.

Greater ATL Health is not an emergency service. In a medical emergency, call 911 immediately or go to the nearest emergency room. Do not use this service for life-threatening conditions.

3. Eligibility

  • You must be at least 18 years old to create an account or book services on your own behalf.
  • A parent or legal guardian may book services for a minor patient.
  • You must be located in the State of Georgia at the time of your telehealth visit. Services are available to Georgia residents only.
  • You must have access to a device with a working camera and microphone and a stable internet connection.

4. Telehealth Limitations

Telehealth services may not be appropriate for all medical conditions. Certain diagnoses, treatments, or prescriptions may require an in-person evaluation. The Provider reserves the right to determine whether your condition can be appropriately treated via telehealth and may refer you to in-person care when clinically necessary.

Controlled substances (Schedule II) cannot be prescribed via telehealth under federal law without an established in-person relationship.

5. Appointments and Cancellations

  • Appointments may be cancelled or rescheduled up to 2 hours before the scheduled start time without charge.
  • Late cancellations (within 2 hours) or no-shows may be subject to a cancellation fee.
  • The Provider may need to reschedule appointments due to technical issues, emergencies, or unforeseen circumstances. You will be notified as soon as reasonably possible.

6. Payment and Fees

Greater ATL Health is a self-pay practice and does not accept insurance. All fees are due at the time of service. Current pricing is listed on our website and may be updated from time to time. You will always be informed of the visit fee before your appointment is confirmed.

We do not offer refunds once a visit has been completed. If you have a concern about a charge, please contact us at hello@greateratlhealth.com.

7. Patient Responsibilities

  • Provide accurate, complete, and up-to-date medical history and information.
  • Inform the Provider of all medications, supplements, and allergies.
  • Disclose relevant health conditions that may affect your treatment.
  • Follow the Provider's care recommendations or communicate your concerns.
  • Contact emergency services in a medical emergency rather than attempting to reach Greater ATL Health.

8. Privacy and HIPAA

Your health information is protected under HIPAA and our Privacy Policy. All video visits are conducted on a HIPAA-compliant, encrypted platform. See our HIPAA Notice of Privacy Practices for full details of your rights.

9. Intellectual Property

Our total liability for any claim shall not exceed the amount you paid for the specific service giving rise to the claim.

12. Governing Law

These Terms are governed by the laws of the State of Georgia. Any disputes shall be resolved in the courts of Fulton County, Georgia.

13. Changes to These Terms
